The seven-time champion lost control on the exit of the Fagnes left-right midway around the lap, in a very similar way to how Alpine’s Pierre Gasly crashed on Friday.
Hamilton’s right-rear wheel smashed into the barrier, leaving the Ferrari mechanics a lot of work to do to repair his car for qualifying at 15:00 BST.
Ferrari say they will be able to complete the necessary work in time.
Mercedes’ Kimi Antonelli set the pace, continuing his impressive form from Friday, with a lap time 0.139 seconds quicker than McLaren’s Lando Norris.
The Italian’s time was set much earlier than the world champion’s. Norris is one of several drivers facing a 10-place grid penalty this weekend for using too many engine parts.
Red Bull’s Max Verstappen was third fastest, 0.148secs back, followed by Antonelli’s team-mate George Russell, who was 0.367secs off the pace.
Hamilton was fifth fastest ahead of team-mate Charles Leclerc.
Antonelli leads Russell by 25 points in the championship heading into the 10th race of the season.
