An inquest has heard that staff falsified records, slept and were on their phones on the night that Hugo Flint-Cahan was killed by another patient. Hugo’s brother, who is an NHS doctor, said there had been a “litany of failures” and blamed “incompetence and dishonesty” on the part of staff. Also on the programme: As tech stocks fall around the world, we ask what the consequences could be for the global economy.
