The BBC has been criticised for broadcasting images of a man critically injured in the 7 July London bombings.
BBC One news and BBC News 24 drew 26 complaints after showing a man receiving heart massage on a stretcher.
Broadcasting watchdog Ofcom censured the BBC, which admitted it had not viewed the footage in its entirety before broadcasting it.
ITV and Channel 4 used the same footage but escaped censure because Ofcom felt they had sufficiently warned viewers.
'Pictures used generically'
The BBC broadcast the footage shortly before 1100 BST on 7 July.
The presenter gave a warning but introduced the images from the hospital by saying: "Let's just take a look at some of the pictures coming from the Royal London (hospital)."
Ofcom said: "It appeared to us that the pictures were used generically and the commentary did not reflect the seriousness of the images being transmitted."
Subsequent evening broadcasts on ITV and Channel 4 attracted a further nine and two viewer complaints respectively.
The BBC said the footage had been transmitted in error and apologised for any offence caused.
Broadcasting code breach
It added the film had been played without being viewed in its entirety because there had been a rush to get material on air.
It said the victim involved "was seriously injured, but not dead" and included an update on his condition later in the day.
